Per\u00f2 com afecta l&#8217;exercici al nostre sistema immunitari?<\/strong><\/p>\n<h2><strong>Beneficis de l&#8217;exercici pel sistema immunitari<\/strong><\/h2>\n<p><strong>Existeix una \u00e0mplia evid\u00e8ncia que mostra que practicar exercici de manera regular enforteix el nostre sistema immunitari<sup>1<\/sup>. Per exemple, diversos estudis han demostrat que la citotoxicitat (\u00e9s a dir, la capacitat per \u201catacar\u201d agents estranys) de les c\u00e8l\u00b7lules Natural Killer (NK, unes de les principals protagonistes del sistema immunitari innat) \u00e9s m\u00e9s gran en esportistes que en persones sedent\u00e0ries o no entrenades.<sup>2,3<\/sup> De fet, participar a un programa d&#8217;entrenament progressiu (per exemple, 4 setmanes realitzant exercici aer\u00f2bic continu a moderada intensitat seguit de 4 setmanes d&#8217;entrenament interv\u00e0l\u00b7lic d&#8217;alta intensitat o HIIT) ha provat augmentar tant el n\u00famero d\u2019aquestes c\u00e8l\u00b7lules<sup>4 <\/sup>com la seva activitat. I aix\u00f2 resultaria en un sistema immunitari m\u00e9s fort. A m\u00e9s, les persones entrenades tamb\u00e9 semblen tenir un sistema adaptatiu m\u00e9s efica\u00e7, cosa que queda palesa, per exemple, despr\u00e9s d&#8217;una vacuna. D\u2019aquesta manera, un estudi recent va mostrar que els esportistes d&#8217;elit presenten una resposta d\u2019anticossos i cel\u00b7lular m\u00e9s gran (major augment de c\u00e8l\u00b7lules T) despr\u00e9s de ser vacunats contra la grip<sup>5<\/sup>. Aix\u00f2 podria ser d\u2019especial rellev\u00e0ncia davant un tema d&#8217;actualitat com \u00e9s la vacunaci\u00f3 contra la COVID-19.<\/strong><\/p>\n<h3><strong>Pot ser perjudicial pel sistema immunitari l&#8217;exercici?<\/strong><\/h3>\n<p><strong>No hi ha dubte que l&#8217;exercici \u00e9s benefici\u00f3s pel sistema immunitari. No obstant aix\u00f2, tradicionalment s&#8217;ha defensat que l&#8217;exercici realitzat de manera extenuant (especialment l&#8217;exercici intens i de llarga durada) podria provocar una certa immunosupressi\u00f3, \u00e9s a dir, una debilitaci\u00f3 del sistema immunitari amb una conseg\u00fcent susceptibilitat a infeccions. Aix\u00ed, per exemple, un estudi que va incloure mostres de m\u00e9s de 2000 esportistes va observar que els esportistes que practicaven m\u00e9s entrenament aer\u00f2bic, com els ciclistes i els triatletes, presentaven un menor nombre de gl\u00f2buls blancs (c\u00e8l\u00b7lules encarregades de la defensa davant processos inflamatoris) que altres esportistes<sup>6<\/sup>. De fet, un 16% d&#8217;aquests esportistes van presentar valors per sota dels de refer\u00e8ncia<sup>6<\/sup>. A m\u00e9s, at\u00e8s que l&#8217;exercici extenuant provoca de manera aguda (1-2 hores m\u00e9s tard) una disminuci\u00f3 transit\u00f2ria de diverses c\u00e8l\u00b7lules immunit\u00e0ries (per exemple, limf\u00f2cits T) a nivell sanguini, s\u00f3n molts els que donen suport a l\u2019anomenada \u201cfinestra oberta\u201d postexercici, \u00e9s a dir, un proc\u00e9s d&#8217;immunosupressi\u00f3 que augmentaria el risc d&#8217;infeccions immediatament despr\u00e9s de realitzar exercici f\u00edsic. De fet, alguns estudis han observat una major prevalen\u00e7a d&#8217;infeccions (sobretot respirat\u00f2ries, com la bronquitis) en esportistes de resist\u00e8ncia en comparaci\u00f3 amb la poblaci\u00f3 general<sup>7<\/sup>, i aix\u00f2 podria potencialment ser degut a un sistema immunitari afeblit per les altes c\u00e0rregues d&#8217;entrenament.<\/strong><\/p>\n<p><strong>Per tant, s\u00f3n molts els que proposen que la relaci\u00f3 entre l&#8217;exercici i la salut del sistema immunitari podria tenir forma de \u201cJ\u201d, sent l&#8217;entrenament a intensitats\/durades moderades benefici\u00f3s, mentre que l&#8217;exercici extenuant podria ser perjudicial per a aquest sistema. \u00c9s important esmentar, no obstant aix\u00f2, que existeix molta controv\u00e8rsia sobre aquest tema. Aix\u00ed, l\u2019evid\u00e8ncia m\u00e9s recent suggereix que l&#8217;exercici no provoca l&#8217;anomenada \u201cfinestra oberta\u201d, sin\u00f3 que la reducci\u00f3 a nivell sanguini de c\u00e8l\u00b7lules immunit\u00e0ries es deu al fet que aquestes c\u00e8l\u00b7lules es traslladen des de la sang cap a teixits perif\u00e8rics en els quals s\u00f3n m\u00e9s importants en aquest moment, com poden ser les mucoses (Figura 1)<sup>8,9<\/sup>. De manera similar, hi ha tamb\u00e9 controv\u00e8rsia respecte la major susceptibilitat a infeccions entre esportistes en comparaci\u00f3 amb la poblaci\u00f3 general, ja que altres factors aliens a l&#8217;exercici en s\u00ed, com pot ser visitar llocs massificats (per exemple, estadis), la nutrici\u00f3, l&#8217;estr\u00e8s, els viatges o alteracions del son, poden afectar tamb\u00e9 aquest risc<sup>9<\/sup>. Per contra, existeix controv\u00e8rsia respecte a si realitzar exercici intens de forma prolongada podria reduir de forma aguda i transit\u00f2ria les nostres defenses, encara que estudis recents posen en dubte aquesta hip\u00f2tesi.
