La quantificaci\u00f3 i el monitoratge de la c\u00e0rrega externa s\u00f3n clau per guiar el proc\u00e9s de readaptaci\u00f3 despr\u00e9s d&#8217;una lesi\u00f3, ja que ens permetr\u00e0 analitzar l&#8217;evoluci\u00f3 de par\u00e0metres relacionats amb la fatiga i realitzar els ajustaments adequats a l&#8217;estat de l&#8217;esportista. El monitoratge de la c\u00e0rrega de treball i un programa de readaptaci\u00f3 efica\u00e7 permetran personalitzar el progr\u00e9s de la rehabilitaci\u00f3, ajudant a optimitzar la presa de decisions per tenir en compte durant el per\u00edode de recuperaci\u00f3 del jugador. L&#8217;\u00e8xit d&#8217;un equip ve en gran part determinat per la disponibilitat per jugar dels jugadors que el componen, per la qual cosa les lesions influiran poderosament en els resultats que l&#8217;equip aconsegueixi al final de la temporada.<\/p>\n<p>El retorn als terrenys de joc o <em>return to play<\/em> \u00e9s un dels aspectes m\u00e9s importants en el proc\u00e9s de recuperaci\u00f3 d&#8217;un futbolista d&#8217;elit despr\u00e9s d&#8217;una lesi\u00f3. El principal objectiu d&#8217;aquest per\u00edode ha de ser preparar el jugador per a la seva reincorporaci\u00f3 a la competici\u00f3 amb el m\u00ednim risc de recaiguda, ja que aquest \u00e9s un moment especialment cr\u00edtic per a l&#8217;aparici\u00f3 de noves lesions. En aquest context, un retorn prematur als entrenaments amb el grup o a la competici\u00f3 augmentar\u00e0 el risc de recaigudes (1, 2). Tanmateix, d&#8217;altra banda, tenir jugadors clau disponibles \u00e9s benefici\u00f3s per a l&#8217;equip. Per aix\u00f2, l&#8217;equilibri risc\/benefici pot arribar a representar un conflicte d&#8217;interessos que requereixi una presa de decisions compartida entre els equips m\u00e8dic i esportiu i el mateix jugador. \u00c9s essencial que comprenguem <em>el return to play<\/em> com un proc\u00e9s constant de presa de decisions, on cada decisi\u00f3 presa condicionar\u00e0 a la seg\u00fcent.<\/p>\n<p>Un programa de readaptaci\u00f3 adequat \u00e9s fonamental de cara a escur\u00e7ar el temps de baixa del jugador perqu\u00e8, a m\u00e9s a m\u00e9s, repercutir\u00e0 en tot el grup en veure&#8217;s compromesa la preparaci\u00f3 en l\u2019\u00e0mbit t\u00e0ctic. Tanmateix, fins ara, l&#8217;efectivitat dels programes de readaptaci\u00f3 descrits a la literatura cient\u00edfica \u00e9s inconsistent. Aquest model es desenvolupa en un marc de 5 fases que van des d&#8217;un elevat control a un elevat caos (<strong>Figura 1<\/strong>). L&#8217;objectiu fonamental \u00e9s tornar al jugador a les c\u00e0rregues de treball pr\u00e8vies a la lesi\u00f3 mitjan\u00e7ant el monitoratge amb GPS.<\/p>\n<p>En una entrevista Taberner ens explica com va concebre el model: &#8220;<em>la meva formaci\u00f3 acad\u00e8mica en Ci\u00e8ncies de l&#8217;Esport, juntament amb anys observant la metodologia de treball en aquest camp dels fisioterapeutes, em va permetre analitzar, processar i identificar \u00e0rees clau que no estaven presents, per\u00f2 sentia que eren necess\u00e0ries dins de la rehabilitaci\u00f3 de lesions en alt rendiment. Una de les principals observacions que vaig extreure va ser que els programes de readaptaci\u00f3 s&#8217;assemblaven poc a les demandes (volum i intensitat) que es manifestaven en reincorporar-se al grup. Tampoc tenien en compte la necessitat d&#8217;un condicionament progressiu que prepar\u00e9s f\u00edsicament el jugador per a les demandes que exigeix el futbol d&#8217;elit en qu\u00e8 se succeeixen les jugades impredictibles que suposen reptes constants per al jugador. "},{"acf_fc_layout":"bloque_de_texto","bloque_de_texto":"<p>En la primera fase, el jugador es troba en un entorn estrictament controlat amb el m\u00ednim risc, movent-se a baixa velocitat i reduint les tasques espec\u00edfiques del joc. Durant la fase de &#8220;Control moderat&#8221;, es comencen a introduir canvis de direcci\u00f3 amb la pilota, per\u00f2 sota circumst\u00e0ncies controlades (en una situaci\u00f3 de caos controlat). Tot seguit, per a la fase de transici\u00f3 entre el control i el caos, s&#8217;inclouen un volum limitat de moviments amb accions imprevistes i aleat\u00f2ries espec\u00edfiques del joc. Durant la 4a fase s&#8217;augmenten les accions a alta velocitat sota un caos moderat (moviments impredictibles), afegint passades i patrons de moviment espec\u00edfics del joc. Finalment, s&#8217;incorporen exercicis dissenyats per executar en escenaris &#8216;ca\u00f2tics&#8217; (velocitats altes\/caos alt) i es desenvolupen elements t\u00e8cnics propis del futbol, per exemple, passar i disparar en moviment. A m\u00e9s a m\u00e9s, en aquesta \u00faltima fase es treballa amb els volums d&#8217;entrenament previs a la lesi\u00f3 i simulant la intensitat de la competici\u00f3.<\/p>\n<p>En aquesta l\u00ednia, s&#8217;acaben de publicar dos casos sobre la implementaci\u00f3 del model <em>control-chaos continuum<\/em> durant la readaptaci\u00f3 i <em>return to play<\/em> de dos futbolistes d&#8217;elit. En un primer cas, despr\u00e9s d&#8217;haver patit una ruptura del tend\u00f3 isquiotibial proximal, el jugador va tornar a la competici\u00f3 120 dies despr\u00e9s de caure lesionat i, despr\u00e9s de 13 mesos, el jugador no va patir cap recaiguda (4). Sorprenentment, el jugador va superar el seu pic m\u00e0xim de velocitat durant el <em>return to play<\/em>. En l&#8217;altre cas, despr\u00e9s d&#8217;una fractura oberta de t\u00edbia i peron\u00e9 distal, el jugador es va reincorporar als entrenaments amb el grup 7,5 mesos despr\u00e9s de la lesi\u00f3 i va completar per primera vegada els 90 minuts d&#8217;un partit al cap de 9 mesos, romanent sense lesions 11 mesos despr\u00e9s del seu retorn (5). Pel que fa a les aplicacions pr\u00e0ctiques del model, el mateix Taberner comenta el seg\u00fcent: &#8220;<em>La rehabilitaci\u00f3 individualitzada d&#8217;alta qualitat \u00e9s un aspecte cr\u00edtic del proc\u00e9s del return to play: un enfocament \u00fanic no aborda l&#8217;\u00e0mplia variabilitat que existeix entre lesions, jugadors, demandes competitives i resposta a la c\u00e0rrega.
