americ\u00e0.<sup>3 <\/sup>Per exemple, en el futbol representen el 37% de totes les lesions musculars.<sup>4<\/sup> A m\u00e9s, un dels principals problemes que suposa \u00e9s la seva elevada taxa de recurr\u00e8ncia, ja que entre el 12 i el 33% dels jugadors que han tingut una lesi\u00f3 d&#8217;isquiosurals pateixen una recaiguda.<sup>5<\/sup> Tot i que en els \u00faltims anys s&#8217;ha incrementat la investigaci\u00f3 i els recursos destinats en aquesta \u00e0rea, la incid\u00e8ncia de lesions en el futbol sembla augmentar (4% per any), tal com mostra un estudi que va analitzar a 36 clubs europeus durant 13 temporades.<sup>6<\/sup><\/p>\n<p>Per aix\u00f2, a causa de la repercussi\u00f3 que suposa tenir un jugador lesionat tant en l\u2019\u00e0mbit esportiu com econ\u00f2mic, recentment s&#8217;ha publicat una revisi\u00f3 narrativa<sup>5<\/sup> on participen entre d&#8217;altres Marc Guitart, membre del departament de rendiment del F.C. Barcelona, juntament amb experts com el Dr. Julio Calleja o el Dr. Pedro E. Alcaraz, on analitzen els possibles mecanismes que determinen la lesi\u00f3 dels isquiosurals.<\/p>\n<p>&nbsp;<\/p>\n<h2>Mec\u00e0nica de cursa. En quina fase existeix major risc de lesi\u00f3?<\/h2>\n<p>En una primera part de la revisi\u00f3, els autors aprofundeixen en els patrons biomec\u00e0nics de la cursa a velocitats subm\u00e0ximes o m\u00e0ximes, fent especial insist\u00e8ncia en els requeriments del cap llarg del b\u00edceps femoral, ja que \u00e9s el principal m\u00fascul afectat en la lesi\u00f3 dels isquiosurals.<sup>7<\/sup><\/p>\n<p>Tal com es veu en la figura 1, el cicle de cursa inclou dues fases: fase de suport (<em>stance<\/em>) i fase de vol (<em>swing<\/em>). J., et al. 2019 "},{"acf_fc_layout":"bloque_de_texto","bloque_de_texto":"<p>Malgrat aquesta evid\u00e8ncia, els autors assenyalen que la majoria dels estudis que investiguen la mec\u00e0nica dels isquiosurals s&#8217;han fet en condicions sense fatiga, encara que \u00e9s un dels principals factors de risc de lesi\u00f3 en els isquiosurals.<sup>13,14 <\/sup> Per aix\u00f2, aquests estudis eludeixen un dels principals mecanismes que incrementen el risc de lesi\u00f3: les alteracions estructurals i funcionals que indueix la fatiga en el m\u00fascul derivades d&#8217;accions amb elevades demandes mec\u00e0niques, com desacceleracions i accions exc\u00e8ntriques.<\/p>\n<p>Aix\u00ed, encara que s&#8217;acostumi a creure que \u00e9s durant el final de la fase de vol quan augmenta en gran manera el risc de lesi\u00f3, els investigadors destaquen <em>&#8220;que aquesta suposici\u00f3 s&#8217;ha establert sense el context global que ofereix una situaci\u00f3 real&#8221;<\/em>. <strong>A m\u00e9s a m\u00e9s, d&#8217;altres investigacions plantegen factors que podrien incrementar el risc de lesi\u00f3 tamb\u00e9 durant la primera fase de suport, ja que existeix una extensi\u00f3 m\u00e0xima del maluc i un pic de for\u00e7a del b\u00edceps femoral durant aquesta fase.<\/strong><sup>15,16<\/sup> Per aix\u00f2, els autors suggereixen que en ambdues fases intervenen elements cr\u00edtics per a la musculatura posterior de la cuixa. Tamb\u00e9 un estudi publicat el 2017 per Lui i col\u00b7laboradors<sup>17<\/sup> suggereix que aquestes dues fases podrien considerar-se com una sola, el que s&#8217;anomenaria per\u00edode de transici\u00f3 de suport-vol (Figura 2), ja que els m\u00fasculs isquiosurals intervenen en l&#8217;extensi\u00f3 del maluc i la flexi\u00f3 del genoll durant tota la fase. Adaptat d'Huygaerts S, et al. 2020"},{"acf_fc_layout":"bloque_de_texto","bloque_de_texto":"<h2>Com es relacionen la fatiga, la funci\u00f3 i l\u2019activaci\u00f3 muscular?<\/h2>\n<p>Durant la cursa a una velocitat elevada en un context de fatiga, si existeix un desequilibri entre la velocitat d&#8217;activaci\u00f3 muscular, el m\u00fascul que s&#8217;activi m\u00e9s o abans tindr\u00e0 major demanda metab\u00f2lica i es fatigar\u00e0 m\u00e9s aviat. En aquest sentit s&#8217;ha observat que a mesura que augmenta la velocitat de cursa, el b\u00edceps femoral s&#8217;activa abans que el semitendin\u00f3s,<sup>10<\/sup> el que crea patrons complexos de coordinaci\u00f3 neuromuscular que poden augmentar la possibilitat de lesi\u00f3.<\/p>\n<p>D&#8217;altra banda, diferents patrons d&#8217;activaci\u00f3 muscular derivats de la fatiga semblen produir canvis biomec\u00e0nics. Per exemple, s&#8217;ha vist que quan es fa un esprint en condicions de fatiga es redueix l&#8217;angle de flexi\u00f3 del maluc i de genoll al final de la fase de vol. Aix\u00f2 produeix una major inclinaci\u00f3 p\u00e8lvica,<sup>18<\/sup> allargant el b\u00edceps femoral durant la cursa, per la qual cosa es fa m\u00e9s vulnerable. Una altra conseq\u00fc\u00e8ncia de la fatiga seria l&#8217;adopci\u00f3 d&#8217;un patr\u00f3 de cursa denominat &#8220;Groucho&#8221;, caracteritzat per una forma de c\u00f3rrer en la que els genolls romanen flexionats al llarg de la gambada.<sup>19<\/sup> Aix\u00f2 provocaria, segons els autors, un augment del cost energ\u00e8tic durant la cursa, reduint-se aix\u00ed l&#8217;efici\u00e8ncia del moviment i una major c\u00e0rrega de les unitats contr\u00e0ctils musculars tant dels m\u00fasculs isquiosurals com de les estructures adjacents (ex. gluti mitj\u00e0), el que augmentaria el risc de lesi\u00f3.
