Aix\u00f2 fa que aquells grups musculars amb una major implicaci\u00f3 en els processos d&#8217;acceleraci\u00f3 i desceleraci\u00f3, com els isquiotibials, siguin m\u00e9s susceptibles de lesionar-se (representen el 12-16% del total de lesions).<\/p>\n<p>Una de les raons per les quals aquesta zona \u00e9s m\u00e9s sensible a lesionar-se podria deure&#8217;s a la composici\u00f3 de les seves fibres musculars. Els isquiotibials tenen una elevada proporci\u00f3 de fibres tipus II o tamb\u00e9 anomenades r\u00e0pides,<sup>1<\/sup> les quals, a causa de difer\u00e8ncies estructurals i metab\u00f2liques respecte de les fibres tipus I, les fan m\u00e9s vulnerables al dany per estr\u00e8s mec\u00e0nic.<sup>2<\/sup> Per aix\u00f2, con\u00e8ixer com responen els tipus de fibres musculars a la c\u00e0rrega de treball podria ajudar a prevenir el risc de lesi\u00f3.<\/p>\n<p>Existeixen an\u00e0lisis de sang tradicionals que mesuren biomarcadors com la creatina cinasa (CK) que relacionen la resposta muscular (en concret, el dany muscular) amb la c\u00e0rrega d&#8217;entrenament en jugadors de futbol,<sup>3<\/sup> per\u00f2 la informaci\u00f3 que aporten \u00e9s limitada i, a m\u00e9s a m\u00e9s, la seva lectura no diferencia entre tipus de fibres. Amb relaci\u00f3 a aix\u00f2, un estudi portat a terme per investigadors de la Universitat de Barcelona,<sup>4<\/sup> va mostrar que la concentraci\u00f3 en sang d&#8217;isoformes r\u00e0pides i lentes de miosina podria ser un marcador fiable de dany muscular indu\u00eft per l&#8217;exercici, podent tamb\u00e9 diferenciar per tipus de fibra i grau. Aix\u00ed, la isoforma de miosina r\u00e0pida seria caracter\u00edstica de les fibres tipus II, mentre que la lenta ho seria de les fibres tipus I. En aquesta mateixa l\u00ednia, Carmona i col\u00b7laboradors han vist que despr\u00e9s d&#8217;un exercici amb un elevat component exc\u00e8ntric, realitzat amb un dispositiu isoinercial, augmenten els nivells de miosina r\u00e0pida, mentre que en esports de resist\u00e8ncia com una ultramarat\u00f3 ho fan els nivells d&#8217;isoforma lenta, el que confereix a la t\u00e8cnica la capacitat de distingir entre tipus de fibres i especialitats esportives.<sup>5,6<\/sup><\/p>\n<p>Un punt a destacar \u00e9s que l&#8217;an\u00e0lisi de les isoformes pot ser sensible a l&#8217;hora de detectar lesions musculars de grau I (petita ruptura de fibres) les quals s\u00f3n dif\u00edcils de diagnosticar a trav\u00e9s de resson\u00e0ncia magn\u00e8tica o ecografia. Tanmateix, un detall a tenir en compte \u00e9s que la t\u00e8cnica tradicional de laboratori (Western blot) emprada per analitzar les isoformes de miosines \u00e9s lenta i no molt sensible, el que no ha perm\u00e8s implementar-la de forma pr\u00e0ctica en l&#8217;esport professional.<\/p>\n<p>Per aix\u00f2, una investigaci\u00f3 portada a terme per membres del FC Barcelona i de la Universitat de Barcelona,<sup>7<\/sup> ha utilitzat una t\u00e8cnica m\u00e9s r\u00e0pida i precisa (ELISA) que els m\u00e8todes tradicionals i ha mesurat els nivells de miosina en sang durant una temporada en l&#8217;Sporting de Gij\u00f3n. En el primer mes de pretemporada van disminuir les seves concentracions (agost) i van augmentar de forma significativa a l&#8217;inici de la temporada (setembre), coincidint amb l&#8217;augment de c\u00e0rrega f\u00edsica. Aquest augment no va ser quantitativament el mateix, ja que aquests tipus de fibra es van danyar en diferent grau, com indica la r\u00e0tio de la taula II (que mostra un major dany de les r\u00e0pides). Els valors de CK i LDH van mostrar una pujada a l&#8217;inici de la pretemporada, i van tornar als nivells basals al llarg d&#8217;aquesta. En aquest cas sembla que aquests s\u00f3n els primers marcadors s\u00e8rics en apar\u00e8ixer quan augmenta la c\u00e0rrega de treball muscular, reflectint un proc\u00e9s inflamatori localitzat en la membrana de la fibra anterior a la disrupci\u00f3 de la mateixa fibra. Si finalment la seva estructura es danya completament, s&#8217;alliberaria miosina a la sang, proc\u00e9s que ja podria mesurar-se amb les isoformes de miosina.<\/p>\n<p>Aix\u00ed mesurar els nivells s\u00e8rics de les isoformes de la miosina pot convertir-se en un procediment que ajudi a millorar els processos de monitoratge de l&#8217;entrenament i la competici\u00f3. Els investigadors afirmen que &#8220;es poden detectar alteracions en l&#8217;estructura del sarc\u00f2mer&#8221;, el que podria ajudar a evitar un dany major durant la temporada.
