\u00e9s la restricci\u00f3 de flux sanguini?<\/h2>\n<p><strong>La restricci\u00f3 de flux sanguini (BFR per les seves sigles en angl\u00e8s <em>[blood flow restriction]<\/em>) ha guanyat una gran popularitat en els \u00faltims anys en el camp de l&#8217;entrenament i la rehabilitaci\u00f3. La BFR consisteix en aplicar pressi\u00f3 (normalment utilitzant un maniguet inflable) a la part proximal dels membres inferiors o superiors per bloquejar el retorn ven\u00f3s sense arribar a bloquejar el flux arterial. Entre d&#8217;altres processos, la BFR ha mostrat augmentar l&#8217;estr\u00e8s metab\u00f2lic, el qual s&#8217;ha proposat com un dels mecanismes que podrien facilitar la hipertr\u00f2fia muscular. De fet, degut possiblement a un augment en l&#8217;acumulaci\u00f3 de metab\u00f2lits i hidrogenions, la BFR ha mostrat estimular l&#8217;alliberaci\u00f3 d&#8217;hormones anab\u00f2liques com l&#8217;hormona del creixement o l&#8217;IGF-1,<sup>1,2<\/sup> aix\u00ed com augmentar la s\u00edntesi proteica muscular.<sup>3<\/sup> \u00a0<\/strong><\/p>\n<h2>Com s&#8217;aplica la restricci\u00f3 de flux sanguini?<\/h2>\n<p><strong>Com ens comentava en <a href=\"https:\/\/barcainnovationhub.com\/ca\/lentrenament-amb-restriccio-del-flux-sanguini-de-levidencia-cientifica-a-la-practica\/\">una recent entrevista<\/a> el Dr. Loennekke (un dels majors experts en el tema a nivell internacional), la BFR ha d&#8217;aplicar-se tenint en consideraci\u00f3 el maniguet que s&#8217;est\u00e0 utilitzant (sobretot la seva amplada, ja que quant m\u00e9s ample \u00e9s el maniguet menys pressi\u00f3 es necessita per restringir el flux sanguini) i l&#8217;individu a qui se li aplica el maniguet (ja que aquells amb extremitats amb una circumfer\u00e8ncia major tendeixen a requerir m\u00e9s pressi\u00f3 que aquells amb extremitats amb una circumfer\u00e8ncia m\u00e9s petita). Malgrat tot, una forma objectiva de controlar la pressi\u00f3 necess\u00e0ria pot ser trobant la pressi\u00f3 d&#8217;oclusi\u00f3 arterial, \u00e9s a dir, aquella pressi\u00f3 m\u00ednima que suposa la p\u00e8rdua de flux arterial completa (quan deixa d&#8217;haver-hi pols a nivell distal), i establir la pressi\u00f3 necess\u00e0ria com un percentatge d&#8217;aquesta pressi\u00f3 d&#8217;oclusi\u00f3 arterial (sent recomanada normalment un 40-80% d&#8217;aquesta pressi\u00f3). Malgrat tot, quan no es t\u00e9 acc\u00e9s a eines com l&#8217;ecografia Doppler per mesurar el pols arterial, en individus sans pot ser suficient determinar la pressi\u00f3 tractant d&#8217;aconseguir un 7 sobre 10 en una escala d&#8217;esfor\u00e7 percebut,<sup>4<\/sup> encara que estar\u00edem perdent precisi\u00f3 i es podria reduir la seguretat i efic\u00e0cia de la intervenci\u00f3.<\/strong><\/p>\n<h2>Beneficis de la restricci\u00f3 de flux sanguini per a la recuperaci\u00f3 de lesions<\/h2>\n<p><strong>A causa de la seva efic\u00e0cia per promoure l&#8217;anabolisme muscular, la BFR \u00e9s cada vegada m\u00e9s usada en el camp de la rehabilitaci\u00f3 per accelerar la recuperaci\u00f3 despr\u00e9s d&#8217;una lesi\u00f3. De fet, una metaan\u00e0lisi publicada a la prestigiosa revista <em>British Journal of Sports Medicine<\/em> que va incloure 20 estudis, va concloure que encara que \u00e9s menys efica\u00e7 que l&#8217;entrenament amb c\u00e0rregues altes, la combinaci\u00f3 de BFR amb exercici de baixa intensitat \u00e9s efica\u00e7 per augmentar la for\u00e7a muscular en pacients en rehabilitaci\u00f3 (per exemple despr\u00e9s d&#8217;una reconstrucci\u00f3 de lligament encreuat anterior, o en pacients amb osteoartritis).<sup>5<\/sup> Per tant, en pacients que encara no poden aixecar c\u00e0rregues molt pesades o realitzar grans esfor\u00e7os, la BFR pot ser una alternativa valuosa. De fet, tamb\u00e9 l&#8217;aplicaci\u00f3 de BFR amb una elevada pressi\u00f3 (~200 mmHg) per si sola (\u00e9s a dir, sense realitzar exercici de forma concomitant) ha mostrat ser efectiva en alguns estudis per atenuar l&#8217;atr\u00f2fia muscular i la p\u00e8rdua de for\u00e7a en pacients immobilitzats despr\u00e9s d&#8217;una operaci\u00f3.<sup>6,7<\/sup> No obstant, sempre que sigui possible la BFR ha de ser aplicada en combinaci\u00f3 amb exercici. De fet, un estudi recent va mostrar que la BFR \u00e9s capa\u00e7 d&#8217;augmentar la s\u00edntesi proteica en aplicar-la simult\u00e0niament amb exercici, per\u00f2 no quan s&#8217;aplicava en rep\u00f2s.<sup>8<\/sup> En aquest sentit, aplicar BFR junt amb exercici aer\u00f2bic a baixa intensitat durant 15 minuts ha mostrat ser tamb\u00e9 m\u00e9s benefici\u00f3s per augmentar la massa muscular i la for\u00e7a, que realitzar exercici aer\u00f2bic sense BFR durant 45 minuts.<sup>9<\/sup> De forma similar, d&#8217;altres autors han observat que l&#8217;aplicaci\u00f3 de BFR de forma simult\u00e0nia amb entrenament de for\u00e7a de baixa intensitat, aporta beneficis semblants als aportats per l&#8217;entrenament de for\u00e7a d&#8217;alta intensitat sense BFR.<sup>10<\/sup> Per tant, la BFR ens permet obtenir guanys similars als obtinguts en exercitar-nos amb c\u00e0rregues altes, per\u00f2 treballant amb c\u00e0rregues baixes. Malgrat tot, sempre \u00e9s convenient realitzar una progressi\u00f3 de l&#8217;est\u00edmul, comen\u00e7ant per l&#8217;aplicaci\u00f3 de BFR en rep\u00f2s fins arribar a la seva combinaci\u00f3 amb exercici de baixa o alta intensitat.<\/strong><\/p>\n<h2><em>Beneficis de la restricci\u00f3 de flux sanguini per al rendiment<\/em><\/h2>\n<p><strong>A m\u00e9s a m\u00e9s dels seus possibles beneficis per a la rehabilitaci\u00f3 de lesions, s&#8217;ha proposat que la BFR podria maximitzar tamb\u00e9 els beneficis de l&#8217;entrenament en esportistes sans. De fet, diferents metaan\u00e0lisis mostren que aquesta estrat\u00e8gia \u00e9s efectiva per augmentar la massa muscular i la for\u00e7a en la poblaci\u00f3 general.<sup>11,12<\/sup> Per exemple, un estudi realitzat en jugadors de Rugbi va mostrar que la realitzaci\u00f3 d&#8217;exercici de for\u00e7a (70%RM) amb BFR augmentava els guanys de for\u00e7a en comparaci\u00f3 amb el mateix entrenament sense BFR.<sup>13<\/sup> A m\u00e9s a m\u00e9s, un altre estudi en individus entrenats va mostrar que la realitzaci\u00f3 d&#8217;esprints al 60-70% de la m\u00e0xima intensitat en combinaci\u00f3 amb BFR, aportava m\u00e9s beneficis en el rendiment en esprint, for\u00e7a i massa muscular que el mateix entrenament sense BFR.<sup>14<\/sup> Per un altre costat, estudis recents mostren que l&#8217;entrenament (pedaleig) amb BFR en persones actives millora el rendiment, la capacitat antioxidant, el metabolisme glucol\u00edtic, el flux sanguini i l&#8217;aportaci\u00f3 i la utilitzaci\u00f3 d&#8217;oxigen en comparaci\u00f3 amb el mateix exercici sense BFR.<sup>15\u201317<\/sup> Malgrat tot, l&#8217;evid\u00e8ncia en esportistes sans \u00e9s m\u00e9s escassa que en el camp de la rehabilitaci\u00f3, i per exemple un estudi realitzat en jugadors semiprofessionals de futbol australi\u00e0 va observar que l&#8217;addici\u00f3 d&#8217;exercici de baixa intensitat amb BFR en un programa d&#8217;entrenament de 5 setmanes que ja inclo\u00efa exercici d&#8217;alta intensitat no aportava beneficis en el rendiment en comparaci\u00f3 amb el mateix exercici sense BFR (Scott, 2017).<\/strong><\/p>\n<h2>Conclusions<\/h2>\n<p><strong>En resum, la BFR es mostra com una estrat\u00e8gia efectiva per maximitzar els guanys de massa muscular i for\u00e7a, podent obtenir amb l&#8217;exercici de baixa intensitat beneficis similars als indu\u00efts amb l&#8217;entrenament amb c\u00e0rregues superiors sense BFR. Aquests resultats s\u00f3n especialment rellevants en el camp de la rehabilitaci\u00f3, on no sempre \u00e9s possible sotmetre els esportistes a altes c\u00e0rregues. A m\u00e9s a m\u00e9s, en esportistes sans l&#8217;addici\u00f3 de BFR a l&#8217;entrenament (ex. durant exercicis de for\u00e7a o tamb\u00e9 esprint) podria maximitzar tamb\u00e9 els beneficis en comparaci\u00f3 amb els mateixos exercicis sense BFR, encara que l&#8217;evidencia sobre aix\u00f2 \u00e9s m\u00e9s escassa.<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p><em>Pedro L.
