Les lesions s\u00f3n una de les grans preocupacions en l&#8217;\u00e0mbit de l&#8217;esport. La baixa d&#8217;un esportista no sols afecta a aquest jugador a nivell individual, sin\u00f3 que t\u00e9 tamb\u00e9 importants conseq\u00fc\u00e8ncies per a l&#8217;equip en veure&#8217;s compromesa la preparaci\u00f3 a nivell t\u00e0ctic, ja que l&#8217;esportista lesionat ha d&#8217;abstenir-se temporalment de competir, la qual cosa repercutir\u00e0 en \u00faltima inst\u00e0ncia en el rendiment de l&#8217;equip.\u00a0Per exemple, el 30% de les lesions esportives suposen entre una i tres setmanes d&#8217;inactivitat. A m\u00e9s, la interrupci\u00f3 en la preparaci\u00f3 de l&#8217;esportista a conseq\u00fc\u00e8ncia d&#8217;una malaltia o d&#8217;una lesi\u00f3 provocar\u00e0 un proc\u00e9s denominat desentrenament. \u00c9s a dir, l&#8217;esportista sofrir\u00e0 la p\u00e8rdua parcial o completa de les adaptacions indu\u00efdes per l&#8217;entrenament (el que inclou guany de pes, p\u00e8rdua de for\u00e7a i massa muscular, i deteriorament de la capacitat cardiorespirat\u00f2ria, entre altres), en resposta a un est\u00edmul d&#8217;entrenament insuficient.<\/strong><\/p>\n<p><strong>Existeix evid\u00e8ncia que per\u00edodes relativament curts, fins i tot dies, sense entrenar s\u00f3n suficients per provocar aquest desentrenament. Aix\u00ed, en persones joves i sanes (~20 anys de mitjana), tan sols s\u00f3n necessaris dos dies d&#8217;immobilitzaci\u00f3 d&#8217;una cama per perdre gaireb\u00e9 un 2% de la massa muscular, mentre que en 5 dies es pot arribar a produir una p\u00e8rdua de massa i for\u00e7a muscular del 3.5% i 9%, respectivament.\u00a0A m\u00e9s, un recent estudi publicat en Plos One va mostrar que dues setmanes sense entrenar durant les vacances post-temporada s\u00f3n suficients per reduir el rendiment en un test intermitent d&#8217;alta intensitat en futbolistes semiprofessionals, encara que no va ser temps suficient per provocar un deteriorament d&#8217;altres marcadors com la for\u00e7a o l&#8217;esprint.6 En una revisi\u00f3 sistem\u00e0tica publicada a Sports Medicine, els autors van concloure que els nivells de for\u00e7a poden mantenir-se fins durant 3 setmanes de desentrenament, per\u00f2 per\u00edodes m\u00e9s llargs (~7 setmanes) resultaran en unes p\u00e8rdues del 14.5% i el 0.4% en els nivells de for\u00e7a i pot\u00e8ncia, respectivament, en jugadors d&#8217;elit de rugbi i futbol americ\u00e0.\u00a0<\/strong><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<h2>Les conseq\u00fc\u00e8ncies del desentrenament secundari a la pand\u00e8mia<\/h2>\n<p><strong>Dins del context actual de pand\u00e8mia en el qual ens trobem, els esportistes tamb\u00e9 han hagut d&#8217;adaptar-se a situacions excepcionals. Aix\u00ed, durant l&#8217;estat d&#8217;alarma que es va proclamar en gran part del m\u00f3n, es van veure obligats a interrompre la temporada havent d&#8217;aparcar la seva preparaci\u00f3 habitual. Encara avui dia s\u00f3n molts esportistes els que han de suspendre els seus entrenaments habituals durant algunes setmanes a causa de la quarantena que han de guardar com qualsevol altra persona. En aquest sentit, els resultats d&#8217;estudis amb models de desentrenament similars a la situaci\u00f3 viscuda durant el confinament \u2013per exemple, la reducci\u00f3 del nombre de passos diaris (\u00e9s a dir, reduir els nivells d&#8217;activitat f\u00edsica di\u00e0ria)\u2013, ens permet obtenir una millor comprensi\u00f3 del deteriorament fisiol\u00f2gic al qual s&#8217;exposen els esportistes durant la quarantena. Aix\u00ed, adults (~36 anys) considerats f\u00edsicament actius (&gt;10.000 passos diaris) que, durant 14 dies, van reduir un 81% el seu nombre de passos diaris i van augmentar ~4 hores el seu temps en sedentari al dia, van mostrar un deteriorament en la capacitat cardiorespirat\u00f2ria i en la massa muscular de les extremitats inferiors, aix\u00ed com en marcadors de salut cardiometab\u00f3lica. D&#8217;igual manera, un estudi que va avaluar l&#8217;efecte de 14 dies en qu\u00e8 un grup de joves va reduir de manera notable els seus nivells d&#8217;activitat f\u00edsica (de 10.500 passos de mitjana a 1.400 passos, similar al que va succeir durant el confinament), va mostrar una p\u00e8rdua del consum m\u00e0xim d&#8217;oxigen del 7% i una p\u00e8rdua de massa muscular de les cames del 3%, encara que no dels bra\u00e7os ni del tronc.9 Aix\u00f2 ha pogut suposar que, com hem vist en un recent estudi sobre les conseq\u00fc\u00e8ncies del confinament en jugadors de b\u00e0dminton d&#8217;elit, els esportistes malgrat seguir entrenant a les seves cases van veure afectat el seu sistema nervi\u00f3s aut\u00f2nom \u2013analitzat a trav\u00e9s de la variabilitat de la freq\u00fc\u00e8ncia card\u00edaca\u2013 i el seu rendiment, amb p\u00e8rdues d&#8217;entre un 6.5% i un 11.5% en diferents tests com el salt o la for\u00e7a m\u00e0xima en esquat, respectivament.\u00a0<\/strong><\/p>\n<p><strong>Per tant, el desentrenament produ\u00eft a conseq\u00fc\u00e8ncia d&#8217;unes poques setmanes d&#8217;aturada esportiva \u2013ja sigui per lesi\u00f3 o malaltia, vacances o confinament domiciliari\u2013 suposar\u00e0 un important pas enrere en la preparaci\u00f3 de qualsevol esportista, i especialment en els d&#8217;elit, no sols afectant el rendiment, sin\u00f3 que comportant molt probablement a m\u00e9s d\u2019aix\u00f2 un alt risc de lesi\u00f3. Per exemple, en diferents esports s&#8217;ha observat que existeix una taxa de lesi\u00f3 entre 2 i 3 vegades m\u00e9s alta durant els entrenaments en pretemporada que durant els de la competici\u00f3,11\u201315 amb el desacondicionament f\u00edsic amb el qual tornen els esportistes despr\u00e9s de les vacances com un dels principals motius d&#8217;aquesta major taxa lesional. Un altre exemple clar \u00e9s el que va succeir despr\u00e9s del <i>lockout<\/i> de m\u00e9s de 3 mesos que es va donar en la Lliga Nacional de Futbol Americ\u00e0 o NFL a conseq\u00fc\u00e8ncia d&#8217;una vaga, durant la qual els esportistes no van poder entrenar de manera habitual.\u00a0En els primers 12 dies de tornada despr\u00e9s de l&#8217;aturada, es van produir 10 lesions del tend\u00f3 d&#8217;Aquil\u00b7les, quan en temporades anteriors es produ\u00efen una mitjana de 5 lesions del tend\u00f3 d&#8217;Aquil\u00b7les durant tota la temporada.<\/strong><\/p>\n<h2><b>Conclusions\u00a0<\/b><\/h2>\n<p><strong>Hem de ser conscients dels importants efectes adversos que se succeeixen a conseq\u00fc\u00e8ncia d&#8217;aturades esportives secund\u00e0ries a una lesi\u00f3, les vacances o el confinament, intentant reduir al m\u00e0xim el cessament d&#8217;exercici f\u00edsic. A m\u00e9s, en els casos en els quals sigui inevitable, aplicarem estrat\u00e8gies nutricionals (major ingesta de prote\u00efna, creatina, HMB, etc.) i f\u00edsiques (entrenament tradicional o, quan per immobilitzaci\u00f3 d&#8217;algun membre no puguem dur-lo a terme, recorrerem a alternatives com l&#8217;electroestimulaci\u00f3, la vibraci\u00f3 o la contracci\u00f3 isom\u00e8trica volunt\u00e0ria) per evitar els efectes negatius associats al desentrenament.<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p><em><span style=\"font-weight: 400;\">Javier S.
